Before you contact us you must ask your insurance company to give us permission to contact them on your behalf.
To allow us to to receive payment directly from your insurer, you will need to book an appointment to come in to our practice with a copy of your insurance details and certificate, including renewal date, excess amount and any exlusions etc. You should also bring with you a signed blank claim form and a copy of our pre authorisation form fully completed and signed - please download a pre authorisation form from here. Once completed we can then start processes your claims.
